Oncology Medical Billing Services
Drug, infusion and radiation billing with unit-level accuracy and pre-treatment authorization control for medical and radiation oncology practices.
Oncology claims carry the highest dollar values in outpatient medicine, which makes small errors expensive: one wrong drug unit, one missing wastage modifier or one unauthorized regimen can cost more than a week of ordinary claims. The specialty also splits across three billing disciplines — drug therapy, infusion administration and radiation treatment — each with its own rules. Elite Bill Care's oncology medical billing services are built to control all three before treatment begins, not after the denial arrives.

Oncology billing specifics
- Drug billing accuracy — antineoplastic and supportive drugs are billed by J-code with units calculated against the exact dose administered, NDC numbers reported as payers require, and vial-size logic checked on every line so a per-milligram code is never billed on a per-vial basis.
- Wastage documentation — discarded drug from single-dose vials is billed with the JW modifier where payers allow it, and the JZ modifier is appended when no drug was discarded, matching the attestation rules payers now enforce.
- Infusion hierarchy — chemotherapy administration, therapeutic infusions, hydration and IV pushes are sequenced under the infusion hierarchy, with only one initial service code per encounter and chemotherapy administration taking its correct precedence.
- Same-day evaluation and management — office visits on infusion days are billed with modifier 25 when the visit is significant and separately identifiable, with documentation that supports the distinction payers look for.
- Radiation oncology billing — treatment planning, simulation, image guidance, IMRT and other delivery techniques, and weekly treatment management are billed in the correct sequence and frequency, respecting the bundling rules that differ from medical oncology.
- Encounter and diagnosis coding — administration encounters are coded with the correct Z-code sequencing alongside the neoplasm diagnosis, and clinical trial services carry the required trial modifiers and registry (NCT) numbers.
Common oncology billing challenges
Authorization is the first exposure: many specialty drugs and radiation courses require prior authorization or step therapy, and a regimen started without one converts directly into uncompensated drug cost. The second is benefit routing — payers increasingly push drugs from the medical benefit to the pharmacy benefit or require white-bagging from a specialty pharmacy, and a practice that buys and bills a drug a payer wanted supplied loses the full acquisition cost. Unit errors are the third: J-code units are dose-specific, and a misplaced decimal is a five-figure mistake. Off-label use adds a fourth, since payment often depends on demonstrating recognized compendia support. Finally, infusion claims fail on sequence — hierarchy violations and missing modifiers generate denials on claims where every service was medically appropriate.
How we run it
Work begins before the first dose. Benefits are verified and authorizations obtained for the planned regimen, and the benefit route — buy-and-bill under the medical benefit or specialty pharmacy under payer policy — is confirmed so the practice never fronts drug cost it cannot recover. Charges are reviewed against the treatment plan and administration record: drugs, doses, units, wastage and infusion sequence are checked as one encounter. High-dollar claims receive senior review before submission, denials are appealed with the clinical record, and drug-related write-offs are reported by cause so patterns are visible at the regimen level.
Benefits of specialized oncology billing
Oncology practices cannot absorb casual billing; the drug economics do not allow it. Elite Bill Care brings more than 10 years of billing experience, a 98% first-pass clean claim rate and denial management that has reduced denials by up to 30% for the practices we serve. Our oncology medical billing services focus on the points where oncology revenue is actually lost — unauthorized regimens, unit errors, wastage that is performed but never billed, and benefit-routing mistakes — protecting both your reimbursement and your drug inventory investment. We serve oncology practices in all 50 U.S. states.
Software and systems we work with
Our team works inside the major EHR and practice management platforms — including athenahealth, eClinicalWorks, AdvancedMD, Tebra, DrChrono, Office Ally, CollaborateMD, NextGen, Practice Fusion and TherapyNotes — including oncology workflows where regimens, orders and administration records originate in the clinical system. All work runs on HIPAA-compliant processes, with drug and dosing data handled as auditable source documentation.

Frequently asked questions
Do you verify benefits and authorizations before treatment starts?
Yes. Pre-treatment financial clearance is a core part of the workflow: we verify coverage for the planned regimen, obtain prior authorizations, and confirm whether each drug is payable under the medical benefit or must be routed through the payer's specialty pharmacy. Starting treatment without that check is how practices end up absorbing drug costs, so it happens before the first administration, not after.
How do you handle drug wastage billing?
When part of a single-dose vial is administered and the rest discarded, we bill the administered amount and the discarded amount on separate lines with the JW modifier where the payer allows wastage payment, and we append JZ when the full vial was used. The amount wasted has to be documented in the administration record, and we flag charts where it is missing so a payable line is not left off the claim.
Can you bill radiation oncology as well as medical oncology?
Yes. We bill both service lines: drug and infusion work for medical oncology, and planning, simulation, delivery and weekly treatment management for radiation oncology. The two follow different bundling and frequency rules, so they are reviewed as separate workflows even when the same patient receives both.
Do you support billing for clinical trial services?
Yes. Routine costs and investigational items on clinical trial claims are distinguished with the appropriate modifiers, and the trial's NCT number is included as payers and Medicare require. We work from your research team's trial documentation so the claim reflects which services are billable to the payer and which are covered by the study sponsor.
