Patient Demographics Entry
Accurate patient and insurance data captured and verified at intake — the foundation every clean claim is built on.
Every claim carries the patient's data with it — name, date of birth, member ID, subscriber relationship, addresses. When that data is wrong, everything downstream fails in predictable ways: eligibility checks miss, claims reject for "patient not found," statements mail to old addresses. Elite Bill Care's patient demographics entry service captures, verifies and maintains this data with the care the rest of the revenue cycle depends on.

What gets captured and checked
- Patient identity — legal name exactly as it appears with the payer, date of birth, gender, address and contact details.
- Insurance details — payer, member ID, group number, plan type, captured from the card (front and back), not from memory.
- Subscriber relationship — self, spouse, child or other, with the subscriber's own details where the patient is a dependent.
- Guarantor information — who is financially responsible, which is not always the patient (pediatrics, dependents).
- Coordination of benefits — primary and secondary coverage recorded in the correct order for patients with two plans.
- Referring and rendering providers — linked correctly for claims that require them.
Why a whole service for data entry?
Because demographics errors are the most common — and most preventable — cause of front-door failures. A single transposed digit in a member ID produces a CO-31 "not an eligible member" denial that looks like a coverage problem and consumes appeal effort. A name spelled differently than the payer's record rejects at the clearinghouse. Our entry process verifies entered data against the insurance card and the eligibility response, so the payer's record and yours agree before the first claim is built on top of them.
Maintenance, not just intake
Patient data decays: people move, change jobs, change plans, marry and change names. Demographics are re-verified when new cards are presented, when eligibility responses contradict the record, and when returned mail or a rejection signals drift — feeding corrections into charge entry and patient billing so statements and claims always use the current record.
Frequently asked questions
What do you need from our front desk to start?
Copies of the insurance card (front and back), the registration/intake form, and the patient's ID details as collected. If intake happens inside your EHR, we work from there — the rule is that entered data is checked against the card image, not typed from memory.
How do you handle dependents and guarantors?
The subscriber and guarantor are recorded as their own fields, never merged into the patient's record — a common source of 'member not found' errors when a child's claim carries the parent's details in the wrong place.
What happens when eligibility contradicts our data?
The payer's response wins, the record is corrected, and the correction is noted — because the same wrong datum would otherwise reject every future claim for that patient. See rejection management for how these patterns are tracked.
Is patient data handled securely?
Yes — demographics work happens inside your systems under a signed Business Associate Agreement, with minimum-necessary access. See our HIPAA page.
