Medical Coding Support
ICD-10, CPT and HCPCS coding review aligned to your documentation, your specialty and each payer's rules — accurate, specific, and compliant.
Coding is where clinical care becomes a claim — and where small inaccuracies become denials, underpayments and audit risk. Elite Bill Care's medical coding support service reviews and supports ICD-10-CM, CPT and HCPCS coding for U.S. practices, aligned to each payer's rules and each specialty's documentation. One rule governs everything we do: we code what is documented — completely, specifically, and never beyond it.

What coding support actually covers
- Diagnosis coding (ICD-10-CM) — coded to the highest specificity the documentation supports: laterality, episode of care, 7th characters where required, and combination codes used correctly.
- Procedure and service coding (CPT) — E/M levels supported by the visit documentation, procedures, time-based services and add-on codes applied in the right order.
- HCPCS Level II — supplies, injectables and services outside CPT, with correct units.
- Modifiers — 25 (separately identifiable E/M), 59 and the X-modifiers for distinct procedural services, telehealth modifiers and place-of-service alignment, and specialty modifiers such as AT in chiropractic.
- Diagnosis–procedure linkage — every procedure supported by a diagnosis that establishes medical necessity, the direct defense against CO-11 and CO-50 denials.
The coding errors that cost practices the most
- Unspecified codes where specificity exists — the single most common avoidable weakness; payers increasingly deny or downcode them.
- Bundling violations — services billed together that NCCI edits bundle, denied as CO-97 or flagged for unbundling.
- E/M levels the note does not support — in either direction: over-coding creates audit exposure, under-coding donates revenue.
- Missing or wrong modifiers — a correct code with a missing modifier denies exactly like a wrong code.
- Outdated codes — deleted ICD-10 and CPT codes still circulating in templates and favorite lists months after the annual updates.
Specialty coding is not generic coding
In mental health, psychotherapy codes are time-based and the documented time decides the code; interactive complexity and add-on rules trip up generalists. In physical therapy, timed units, the 8-minute rule and evaluation complexity levels decide payment. In pediatrics, preventive visit coding by age and vaccine administration codes carry their own logic. Our coding support works within your specialty's rules — browse the full list on our specialties page.
How the workflow runs
- Documentation received from your EHR as part of charge entry.
- Review and coding — diagnoses, procedures, units and modifiers assigned and checked against payer rules for your specialty.
- Provider queries when documentation is ambiguous or incomplete — specific questions, not generic bounce-backs.
- Edits before submission — bundling, linkage and validity checks run before the claim leaves through claim submission, so fixable denials never ship.
- Feedback loop — patterns found in denied claims feed back into coding review, closing the loop inside the full revenue cycle.
Compliance is the service
Any coder can make a claim pay once by coding aggressively; the cost arrives later as audits, recoupments and payer investigations. Our support is deliberately conservative: documentation first, code second, payer policy third — and when the three disagree, the documentation wins and the provider hears about the gap. That stance protects the practice while still capturing every dollar the record legitimately supports.
Frequently asked questions
Do you code from our documentation, or change codes to get claims paid?
We code only what your documentation supports — never beyond it. Upcoding and unbundling are compliance violations that create audit and repayment risk far larger than any single claim. Our job is the opposite: make sure the documented care is translated into the most accurate, specific codes available, and flag documentation gaps back to the provider so the record and the claim agree.
Which code sets do you work with?
ICD-10-CM for diagnoses, CPT for procedures and services, and HCPCS Level II for supplies, drugs and services CPT does not cover — including the modifiers each payer expects (such as 25, 59 and the X-modifiers, telehealth modifiers, and specialty-specific ones like chiropractic's AT).
How do you keep up with code changes?
ICD-10-CM updates take effect each October 1 and CPT updates each January 1, alongside quarterly HCPCS changes and payer policy bulletins. Code review is built into our workflow so deleted and revised codes stop appearing on your claims when they change.
Can you help with a specialty you have not billed before?
We will tell you honestly. Our coding support is strongest across the specialties on our specialties page. For a field outside that experience we will say so in the consultation rather than learn on your claims.
What happens when documentation is not enough to code from?
We query the provider with a specific question — what is missing and why it matters — rather than guessing or holding the claim silently. Recurring gaps become a short documentation feedback note, so the same query stops repeating.
