Claim Scrubbing Services
Every claim checked against payer rules before it is submitted — eligibility, coding, demographics and authorizations verified up front, so claims are accepted the first time instead of returned for repair.
A claim error found before submission costs a minute. The same error found by the payer costs a rejection, a resubmission, or a denial letter and an appeal cycle. Elite Bill Care's claim scrubbing services put a disciplined review in front of every claim: automated edits run against each charge, and a human reviewer resolves what the edits flag, so what leaves your practice is a claim that has already survived the checks that stop most claims at the door.
What our claim scrubbing checks
Scrubbing is not a spell-check. Each claim is tested against the rules that actually stop claims, in the order payers apply them:
- Eligibility and coverage — active coverage on the date of service, correct payer and plan, and coordination-of-benefits order, so claims are not built on a policy that was never in force
- Demographics and member data — patient name, date of birth, member ID and subscriber relationship matched to the payer's record, the single most common source of front-end rejections
- Coding accuracy — procedure and diagnosis codes valid for the date of service, diagnosis pointers consistent with the services billed, and code combinations screened against bundling rules
- NCCI and modifier edits — procedure-to-procedure bundling, medically unlikely units and the modifiers that must be present — or must not be present — for the pair to pay
- Authorization on file — services that require prior authorization checked against an approval that matches the date, the provider and the service billed
- Provider and enrollment data — rendering and billing identifiers, taxonomy and enrollment status correct for the payer the claim is routed to
- Duplicates and frequency — repeat submissions of the same service and frequency limits screened before they become duplicate denials
Rules engines catch patterns; people catch context
Scrubbing software is fast and literal. It will flag a missing modifier every time, and it will also flag claims that are perfectly correct — a modifier combination that is unusual but documented, a second procedure that looks bundled until the operative note shows separate sites. A rules engine cannot read a chart.
That is why our claim scrubbing services pair the edit engine with human review. The engine screens every claim at volume; a reviewer works the exceptions, reads the documentation behind the unusual claim, and either clears it with the supporting detail or sends it back for correction before submission. The feedback runs both ways: when the same flag fires repeatedly for a legitimate pattern, the rule set is tuned; when it fires for a real error, the correction is made at the source — in the charge entry, the eligibility step or the authorization workflow — so the error stops recurring.
Scrubbing, rejections and denials are three different events
These terms are often used interchangeably, and the difference matters. Scrubbing happens before submission, inside your own process: an error caught here is corrected and the claim goes out clean. A rejection happens at the clearinghouse or the payer's front door: the claim never entered adjudication, earns no denial letter and starts no payment clock — it must be corrected and resubmitted, and it is the event our clearinghouse rejection management service works when scrubbing or an upstream system lets one through. A denial is an adjudicated refusal with a reason code, worked through correction or appeal under denial management.
Scrubbing is the cheapest of the three by a wide margin. Every error it catches is an error that never consumed payer turnaround time, never aged in a rejection queue and never needed an appeal.
How scrubbing fits your existing systems
Scrubbing sits between charge entry and submission in the systems you already run. Our team works inside your EHR and practice management platform and through your clearinghouse: charges are reviewed in your workflow, edits are resolved against your documentation, and accepted claims flow to submission without a system change or a data migration. Scrubbing rules are configured to your payer mix and specialties — a behavioral health practice and a surgical group do not stop on the same edits — and every step runs on HIPAA-compliant processes with named-user access to your systems. The service operates for practices in all 50 U.S. states.

Frequently asked questions
How is claim scrubbing different from what our clearinghouse already does?
Clearinghouse edits check format and basic validity — whether the claim is built correctly as a file. Scrubbing checks whether the claim is correct as a request for payment: coverage in force, codes that support each other, bundling and modifier rules, authorization on file. The two layers catch different errors, and scrubbing runs before the clearinghouse ever sees the claim.
Does scrubbing slow down claim submission?
No — it protects the submission timeline. Automated edits run as charges are finalized, and flagged claims are reviewed the same day, so clean claims go out on schedule. The alternative is slower by weeks: a rejected claim that waits in a queue, or a denial that has to be appealed.
What happens when scrubbing flags one of our claims?
The claim is held and the issue is fixed at its source — a corrected member ID, a missing modifier added from the documentation, an authorization obtained or matched — then released for submission. You see held claims and the reasons in reporting, so nothing is delayed silently and recurring causes can be fixed in the workflow that produced them.
Can scrubbing really reduce our denials?
Yes, because a large share of denials are front-end errors wearing denial codes: wrong member data, missing authorizations, bundling and modifier mistakes. Catching them before submission is a direct reason our clients maintain a 98% first-pass clean claim rate and achieve up to 30% denial reduction. Scrubbing cannot fix a service that was never covered, which is why it works alongside eligibility verification rather than replacing it.
Put a Check in Front of Every Claim
If your team learns about claim errors from rejection reports and denial letters, the errors are being found in the most expensive place possible. Request a free consultation and we will review a sample of your recent rejections and denials and show you how many a scrubbing step would have caught before they left the building.
